Effective solutions to stop posthemorrhagic syndromes

The rehabilitation on the basis of syndrome differentiation is needed due to dysfunctions of viscera and tissues and complicated symptoms after various types of hemorrhage like hemoptysis, haematemesis, hematuria, hematochezia, epistaxis, gingival hemorrhage, hematohidrosis, metrorrhagia and metrostaxis.

1. Syndrome differentiation and treatment
(1) Deficiency of yin and body fluids
It manifests hectic fever, flushing of zygomatic region, night sweat, dry mouth and throat, dizziness, tinnitus, lumbar soreness, flaccidity of knees, restlessness due to asthenia, inadequate sleep, constipation, dry skin, red tongue with a litte fur, thready and rapid pulse. It is advisable to nourish yin and body fluids. In the case of pulmonary yin deficiency and concurrent dry cough and scanty sputum, use the modified Baihe Gujin Decoction (Lily Decoction for Strengthening the Lung). In the case of thirst with inclination to drink due to pulmonary heat and yin deficiency, use the modified Yunv Decoction (Gypsum Decoction); in the case of soreness and flaccidity of waist and knees due to hepatorenal yin deficiency, use the modified Liuwei Dihuang Bolus (Bolus of Six Drugs Including Rehmannia) in combination with Pulsation-Restoring Decoction.

(2) Deficiency of qi and blood
It manifests pale or sallow complexion, light-headedness, palpitation, short breath, lassitude, hypodynamia, poor appetite, numbness of hands and feet, pale lips and tongue, thready and weak pulse. It is advisable to supplement qi and nourish blood. In the case of deficiency of qi and blood, use the modified Maidong Yangrong Decoction; in the case of cardiosplenic asthenia, use the modified Guipi Decoction (Decoction for Invigorating the Spleen and Nourishing the Heart); in the case of consumptive diseases and sallow complexion, use the modified Guiqi Jianzhong Deoction(Decoction of Radix Angeticae Sinensis and Radix Astragali seu Hedysari for Strengthening the Middle Energizer). (3) Internal retention of blood stasis
It manifests general or local pain, or headache, or thoracic and hypochondriac stabbing pain with definite site, or low fever at dusk, restless nocturnal sleeping, blackish eyelids, cyanotic tongue with ecchymoses, thready and unsmooth pulse. It is advisable to activate blood circulation to remove blood stasis and promote generation of new blood. The prescription used is the modified Xuefu Zhuyu Decoction (Decoction for Removing Blood Stasis in the Chest).

It is also advisable to select Resuscitation-Inducing and Blood-Activating Decoction, Decoction for Dissipating Blood Stasis under Diaphragm, Decoction for Removing Blood Stagnation in the Lower Abdomen and Decoction for Relieving Pantalgia due to Blood Stagnation according to the different sites of blood stagnation. In the case of concurrent qi deficiency or blood deficiency, use the modified Buyang Huanwu Decoction (Decoction of Invigorating Yang for Recuperation) or Taohong Siwu Decoction respectively. 2. Rehabilitation methods
(1) Regulating emotions
Guide the patients to learn how to regulate and relax themselves and establish the good psychological state.

(2) Recreation therapy
As for those who tend to be emotionally stable, guide them to participate in recreational activities such as appreciating music, calligraphy and painting, practicing calligraphy, reciting and composing poems, growing flowers and raising birds. The musical therapy: for the patients with mental stress and anxiety, select the tunes of tranquilizing the mind; for the patients with mental depression, select the tunes of removing mental depression; for the patients with susceptibility to excitement and anger, select the sorrowful tunes. (3) Acumox and massage
Acumox: In the case of deficiency of yin and body fluids, select Ganshu (BL 18), Shenshu (BL 23), Neiguan (PC 6), Taixi (KI 3) and Sanyinjiao (SP 6) and needle them with the reinforcing manipulation. In the case of insomnia, add Shenmen (HT 7); in the case of concurrent residual heat, add Feishu (BL 13), Dazhui (GV 14) and Hegu (LI 4) and needle with them the uniform reinforcing-inducing manipulation.

In the case of deficiency of qi and blood, select Zusanli (ST 36), Pishu (BL 20), Guanyuan (CV 4), Qihai (CV 6) and Baihui (GV 20) with the reinforcing manipulation, and moxibustion is applicable additionally; in the case of poor appetite, add Zhongwan (CV 12) and Weishu (BL 21); in the case of internal accumulation of blood stasis, add Qimen (LR 14), Geshu (BL 17), Qihai (CV 6), Xuehai (SP 10) and Taichong (LR 3) with the reducing manipulation, and the mild moxibustion is applicable to the local blood stasis.

Massage: Mainly use the health care massage. In addition to massaging the waist, back and abdomen, digitally press the acupoints in the light of the acupoint selection in acumox, and mainly press and knead the acupoints of the five zang organs as well as Zusanli (ST 36), Sanyinjiao (SP 6) and Xuehai (SP 10). It is advisable to use gentle and slow manipulations. The massage lasts for about thirty minutes. Ten to fifteen times constitute one course of treatment in general.

(4) Dietary therapy
In the case of deficiency of yin and body fluids, select Gruel of Huangjing (Rhizoma Polygonati); in the case of pulmonary yin deficiency, select Gruel of Baihe (Bulbus Lilii); in the case of gastric yin deficiency, select Gruel of Maimendong (Radix Ophiopogonis); in the case of renal yin deficiency, selcect Gruel of Shanyurou (Fructus Corni); in the case of deficiency of qi and blood, select Buxu Zhengqi Gruel (Deficiency-Curing and Qi-Strengthening Gruel); in the case of internal accumulation of blood stasis, select Gruel of Yimucaozhi (Juice of Herba Leonuri) or Gruel of Taoren (Semen Persicae)respectively according to the specific state of disease.
