Kexue Fang, Prescription for Treating Hemoptysis

Ingredients:
No. 1 Qingdai (Indigo Naturalis) (ground in water) 6 g;
No. 2 Gualouren (Semen Trichosanthis) 9 g;
No. 3 Hezi (Fructus Chebulae) 6 g;
No. 4 Haifushi (Pumex) 9 g;
No. 5 Zhizi (Fructus Gardeniae) (parched black) 9 g.

Administration:
Grind the above drugs into fine Powder and make it into pills with honey and ginger juice; dissolve in the mouth 9 g each time and twice daily. Or decoct the above drugs with proper dosages in proportion to the original recipe.
 
Actions: Expelling fire from the liver and lung, eliminating heat from blood and arresting bleeding.
 
Clinical Application:
This recipe is indicated for hemoptysis due to invasion of the lung by liver-fire, marked by cough, thick sputum with blood streaks, difficult expectoration, vexation and irritability, chest and hypochondriac pain, dry throat and bitter mouth, flushed face, constipation, reddened tongue with yellow fur, taut and rapid pulse. It is applicable to hemoptysis as seen in bronchiectasis and tuberculosis, which pertains to liverfire invading the lung. In case of yin impairment by fire, add Shashen (Radix Adenophorae Strictae ) and Maimendong (Radix Ophiopogonis) to moisten the lung and nourish yin.

In case of severe cough with much sputum, add Zhebeimu (Bulbus Fritillariae Thunbergii), Tianzhuhuang (Concretio Silicea Bambusae) and Pipaye (Folium Eriobotryae) to remove heat from the lung so as to resolve phlegm and relieve cough. In case of bleeding in large amount, add Xianhecao (Herba Agrimoniae) and Baimaogen (Rhizoma Imperatae) or use this recipe together with Shihui San.
  
Elucidation:
The syndrome results from liver-fire scorching the lung, which leads to impairment of the lung collaterals, and is known as "Wood-fire impairs metal". It should be treated by clearing away heat from the liver and lung and eliminating heat from blood to arrest bleeding. Ingredient No. 1 can eliminate heat from the liver and blood and No. 5 purge away fire to relieve vexation, both used as monarch drugs to purge away the liver-fire. No. 2 can clear away heat and dissolve phlegm, moisten the lung and relieve cough, while No. 4 clears away heat from the lung and eliminates phlegm, both functioning as minister drugs.

No. 3 has an astringent effect on the lung and acts as adjuvant drug. Although this recipe is named "recipe for treating hemoptysis" in Chinese, it comprises none styptic drug. Thus it shows that this recipe is formulated in light of the principle of purging away liver-fire and removing heat from the lung to relieve cough so that bleeding will be arrested.

Cautions:
This recipe is contraindicated for hemoptysis due to yin deficiency of the lung and kidney or that concomitant with loose stool due to spleen deficiency.
